Parents and legal guardians of students new to the district will be asked to scan and upload the documents listed below. Please have these ready before beginning the electronic enrollment process. If you are unable to provide any of the required documents, contact the school that serves your home address for assistance.

PHOTO IDENTIFICATION OF PARENT/LEGAL GUARDIAN

Accepted forms include: Driver’s License or State ID, Matricula Consular, Work Authorization Card, Border Crossing Card, Permanent Resident Card, Passport, or Visitor Visa.

TWO PROOFS OF RESIDENCE (ONE FROM EACH SOURCE)

Source 1: Deed, current lease, closing/settlement statement, or property tax statement showing the property is owned or rented by you and located within the school’s attendance boundary.

Source 2: Current gas, water, or electric bill/activation dated within the last 30 days; must show both the service address and mailing address.

CHILD’S BIRTH CERTIFICATE

Only parents or legal guardians may register a student. Legal guardians must provide court-ordered custody or guardianship documents. Notarized statements and powers of attorney cannot be accepted.

CHILD’S IMMUNIZATION RECORDS

All required immunizations must be submitted within 30 days of the student’s start date. Students missing required immunizations after 30 days will not be permitted to attend until compliant.

If your child already has up-to-date immunizations, please scan and email the records to your school’s registrar or enrollment liaison.

HEALTH ASSESSMENT K–12

Effective July 1, 2016, every student entering a North Carolina public school for the first time must submit a Health Assessment within 30 days of the first day of attendance.

If your child has had a physical or well-check in the past year, your healthcare provider can complete the NC Health Assessment form. Please scan or forward the completed form to your school’s registrar or enrollment liaison as soon as possible.

You must submit this form within 30 calendar days from the start of school.
Example: If school begins on May 18, the form is due no later than June 17.
